Online Forms & Surveys
Build a Google Form with different question types, share it by link or QR code, collect answers and send them to a spreadsheet.
⌨️ Start with your 10-minute typing warm-upBy the end, you will be able to:
- Build a Google Form with a title, a description and the right question type for each question (short answer, paragraph, multiple choice, checkboxes, drop-down, linear scale, date).
- Use required questions, sections, response validation and the response settings, preview the form, and share it by link or QR code.
- Read the Summary charts, link the responses to Google Sheets, and explain how to ask fairly: ask only what you need, say why you are asking and keep answers private.
What you need
No computer at home? Print the PDF and practise on paper.
Follow these steps in order
Tick each step when you finish it. Your ticks are saved on this device, so you can stop and come back later.
Learn the ideas ⏱️ 35 min
Read pages 1–4 of the workbook and try every small task. Type your answers straight into the boxes: they save by themselves.
Explore the 3D lab ⏱️ 15 min
Play with the 3D model below. The 🎯 missions tell you exactly what to do next. When you finish every mission, this step ticks itself.
Do the Lab Mission ⏱️ 25 min
Page 5 is the hands-on task. Do it on a real computer, step by step. You need: internet and a Google account (the Hub's learning account).
Check yourself ⏱️ 10 min
Answer the questions on page 6 without looking back, then check your answers below.
Finished? Check your answers
Pick the Best Type (Page 2)
- a) Checkboxes (they may enjoy several).
- b) Drop-down (a long list of 16 regions; multiple choice is also acceptable).
- c) Linear scale (1 to 5).
- d) Multiple choice (Yes / No).
- e) Paragraph.
Your turn (Page 4): for example "Half of the pupils (10 of 20) walk to school" or "Walking is the most common way to get to school, then trotro or bus (a quarter)."
Lab check (Page 5): 6 question types (drop-down, multiple choice, short answer, checkboxes, linear scale, paragraph). Typing 500 shows the error message and the form cannot be submitted. Responses and the most popular food vary. The linked spreadsheet's default name is the form title followed by "(Responses)".
True or False (Page 6)
- Checkboxes let people choose more than one answer: True.
- A food survey should ask for full names and phone numbers: False (ask only what you need).
- Response validation can stop someone typing 5000: True.
- It is fine to post a screenshot of one classmate's answers: False (keep answers private; share totals only).
- Link to Sheets puts every response into a spreadsheet, one row per person: True.
Written Review Questions
- With choices, everyone's answer is spelt the same way, so the form can count them correctly and draw a clear chart. With short answer people type different spellings ("jollof", "Jollof rice"), which are counted as different foods. It is also faster to answer.
- Any neutral version, for example "How would you rate the canteen food?" (linear scale 1 to 5) or "What do you think of the canteen food?" It must not suggest the answer.

Stuck? Try this
Most problems are one missed word. Look for the Panic Button or Help box in the workbook.
Refresh the page. On a very old computer it may not show 3D. The workbook still has everything you need.
Answers save on the computer you typed them on. Open the workbook on that same computer and browser.
Ask your mentor, a teacher or a friend. Show them the step number you are on.
